Ohio mental health counselor Jennifer Dean reprimanded by state board

On December 5, 2011, the Ohio Counselor, Social Worker & Marriage and Family Therapist Board reprimanded professional clinical counselor Jennifer A. Dean for failure to maintain appropriate standards of care with regard to her client records, in that she either did not create/complete them or did not print them out as required, causing lengthy delays in the continuity of care of clients. In addition to reprimand, Dean must undergo face-to-face monitoring of her practice by a Board-approved supervisor at her own expense, as well as obtaining additional continuing education.
