Texas takes disciplinary actions against counselors, October 2012

The Texas Department of State Health Services reported that on October 26, 2012, the Texas State Board of Examiners of Professional Counselors took the following disciplinary actions against licensed professional counselors:

Reprimanded Ronald Edwards, relative to his failure to release client medical/psychological treatment records, upon written request.

Ordered Lexey Fender-Sadler to pay an administrative penalty of $1,500 related to her failure to release the confidential treatment records in a timely manner to the mother of a client.

Reprimanded John Gorman, related to his failure to set and maintain professional boundaries by engaging in a dual relationship with a client.

Reprimanded Donna Granato for failure to set and maintain professional boundaries by engaging in a dual relationship by counseling the children of her office manager.

Reprimanded Glen Jennings for failure to keep accurate client records.

Reprimanded Frances Paulette Lee for failure to set and maintain professional boundaries by engaging in unethical conduct that included bartering for services with a client’s mother.

Placed Chales (Charles?) Miears on two years probated suspension for failure to set and maintain professional boundaries by entering into a dual relationship with client, engaging in unethical conduct by knowingly over-treating a client.

Placed Mary Mulcahy on two years probated suspension for failure to set and maintain professional boundaries by entering into a dual relationship with client and failure to provide
treatment records to client.

Reprimanded Catherine Ranum for engaging in false, misleading and deceptive advertising on website, Accepted payments from clients as an LPC-Intern and failure to submit required applications to the Board.

Revoked the license of Laura Walker for failure to set and maintain Professional boundaries by entering into a dual/sexual relationship with client, failure to keep accurate client records, failure to maintain client records, and failure to keep board file updated.

Reprimanded counselor Ruth Yeh, for providing supervision as a Licensed Professional Counselor Supervisor after his supervision status had expired.
