State reprimands psychiatrist Herbert Grossman on presribing issues

On December 6, 2012, the Arizona Medical Board reprimanded psychiatrist Herbert M. Grossman for treatment of a patient which fell below the standard of care in several ways, relative to the prescribing of opioid drugs and benzodiazepines. The Board's document states that, among other things, Grossman failed to conduct a comprehensive assessment of substanvce use and that there were no urine drug screens in the patient's chart; failed to the patient enter into a treatment agreement and he failed to involve other adjunctive treatments, including 12-step program or chemical dependency support/relapse prevention groups.
