Texas mental health counselor disciplinary actions, November 2013

On November 19, 2013, the Texas State Board of Examiners of Professional Counselors (Board) recently took the following disciplinary actions:

Ordered licensed professional counselor Alan Apperson to pay a $500 administrative penalty for failure to provide the required supervisor agreement form.

Ordered licensed professional counselor Joyce Ratner to pay a $500 administrative penalty for failure to provide the required supervisor agreement form.

Ordered licensed professional counselor Julius Lundy to pay a $1,500 administrative penalty for failure to provide the required supervisor agreement form.

Ordered licensed professional counselor Shannon Johnson to pay a $500 administrative penalty for failure to provide the required supervisor agreement form. 

Ordered licensed professional counselor George Pate to pay a $500 administrative penalty for failure to provide the required supervisor agreement form.

Ordered licensed professional counselor Barbara Abramowitz to pay an administrative penalty for failure to provide the required supervisor agreement form.

Placed licensed professional counselor Sandra Fleming on one year of probated suspension for failure to report her criminal history to the Board.

Reprimanded licensed professional counselor Ryan Crawford for failure to keep accurate client records.

Placed licensed professional counselor Jeana Herndon on five years probated suspension for having engaged in a dual relationship with a client.

Revoked the license of licensed professional counselor Ronald Hodgkiss due to a felony conviction.

Accepted the voluntary surrender of licensed professional counselor Alysius Allen’s license due to failure to report to the Board a previous disciplinary action from another state licensing board.
