Psychiatrist Jeffrey Harazin Loses Medical License; Initiated Sex with Patient

On November 17, 2017, the Colorado Medical Board accepted the relinquishment of psychiatrist Jeffrey Harazin's license to practice medicine. 

The Board's Stipulation and Order states that Dr. Harazin admitted that he'd engaged in sexual encounter with a patient whom he was supposed to be provideing care and assistance in recovering from an addiction to opioids. The sexual encounters, which occurred six times, included intercourse and oral sex. Dr. Harazin admitted to initiating all such encounters.

I would like to know how to go about getting medical records from Doctor Jeffrey S. Harazin, Psychiatrist. in 2017, Jeffrey S. Harazin, M.D. had a stipulation and final agency order in the matter of the disciplinary proceeding regarding the license to Practice Medicine in the State of Colorado License Number DR-36805.
