Arizona psychiatrist reprimanded for patient record keeping failure

The Arizona Medical Board publicly reprimanded psychiatrist Beatrice Yang for unprofessional conduct for failing or refusing to maintain adequate records on a patient. The Board found that Dr. Yang was terminated from employment by a mental health care provider because she added data to the records of a deceased patient (victim of suicide) after she had received a request for completion of a questionnaire from a life insurance company. She did not indicate that the information was added after the receipt of the request of completion of the questionnaire. The records lacked contemporaneous progress notes and prescription information.
