Nexus East Bethel Family Healing: More Than Three Dozen Violations in Less Than Three Years

Minnesota’s Department of Human Services has ordered the closure of Nexus East Bethel Family Healing, a psychiatric residential treatment facility for youth, after documenting a pattern of violations and determining that conditions posed an imminent risk to residents. The facility, which opened in 2023 and served children and teens with serious mental health needs, had repeatedly been cited for deficiencies before the state imposed a temporary immediate suspension.

According to reports, state investigators identified numerous problems involving supervision, treatment planning, staffing, and the protection of vulnerable youth. Previous investigations included substantiated findings of neglect and repeated correction orders, raising concerns about the facility's ability to provide safe care.

The closure of Nexus East Bethel Family Healing highlights ongoing concerns about oversight and accountability in youth behavioral health programs. State officials said the action was necessary to protect the health and safety of the young people placed in the facility.
