Holly Hill Hospital in East Raleigh has experienced repeated incidents of adolescent patients escaping its locked psychiatric units, including seven teens who fled in September 2025 after assaulting a security guard. State inspections have documented 27 deficiencies since January, including staff shortages, inadequate supervision, unsecured medication rooms, and failures to report incidents such as self-harm. One 14-year-old patient injured herself using staples from a worksheet packet despite being on close watch. In another case, a group of girls breached a nurses’ station, damaged equipment, and accessed syringes.
