Elysium Healthcare: Coroner Inquest Finds Staff Falsified Medical Record, Failed to Monitor Suicidal Patient

A coroner’s inquest identified a “dangerous culture” at the Farndon Unit in Newark, operated by Elysium Healthcare, following the 2022 death of 20-year-old patient Molly-Star Kirk, citing systemic failures including missed required five-minute observations, falsified monitoring records, inadequate staff training, and weak management oversight. Evidence showed staff deliberately completed observation logs to appear compliant despite CCTV indicating long periods where checks were not performed, with only 51 of at least 108 required observations completed overnight before her death.
